Please note participants have one year from the final day of the workshop to submit.

The following information and documents must be submitted to complete Presenters Forum.


NOTE: Be sure to have all information and documents ready to go; you will not be able to save progress and return to the form later.


  • Presenters Forum certificate

  • Presenting Video*

  • Completed Video Template

  • Completed Presentation Skills Self-Assessment

  • Written Self-Reflection


*Video files are not accepted. Upload videos to YouTube or a shared drive space to obtain a shareable link.
